- In just a few words, how is the LTC industry changing today?

The industry is evolving and facilities have much more responsibility for the rehabilitation of patients, as they are transferred out of a hospital following an event, rehabilitating them so they can ultimately be moved through assisted living or back to home care. The length of stay on a high percentage of patients is less because of that, so the pharmacy dispensing cycle should also be shorter.

- Why do LTC facilities need an "in-house pharmacy"?

Most patients are on 13 to 18 different medications, prescribed by a number of different doctors. Facilities must ensure that patients are taking the right medication at the right time, but also review the orders to ensure they are therapeutically correct based on the diagnosed conditions and that there are no adverse interactions between the various prescriptions. For all information to be up-to-date, prescriptions must be managed electronically. Getting away from inefficient paper processing of prescriptions would also give nurses more time to focus on patient care.

- How could they lower the per patient per day cost?

This is an area where we play a critical role. Many products are now available generically, and we try to maximize the dispensing of generic drugs. We also work with the facilities around compliance on medications and the prevention of adverse reactions to avoid the patient having to be readmitted to the hospital, which drives up healthcare costs.

- What opportunities do facilities underutilize today? What could they capitalize on?

The pharmacy plays a big role around the star rating for Centers for Medicare and Medicaid Services (CMS). Ensuring the facility is adhering to state and federal regulations, positioning it to a higher level of reimbursement through its CMS ratings, that is an area they could look into.

- How could they maximize reimbursement?

Reimbursement can be maximized through the use of specialty pharmacy medications. There are more and more specialty pharmacy products available, which are more expensive than other products. As we look to the future, specialty pharmaceuticals will get us into diagnostic testing and personalized medicine. Thus in a treatment for oncology, if specialty products are available, we will be using more diagnostic tests, so the physician can prescribe a product that is more effective for the specific patient.
